If the axle nuts tight and you can still move the wheel it sounds like the thread on axle and axle nut is siezed and binding. I wouldn't be in a hurry to ride it until fixed.

On re reading your post I gather the gap is between adjuster block and adjuster ? ? ? If so then I'd say the adjuster has come loose and vibrated itself back to create the gap as the wheel would pull forward not back if loose then I'd simply check wheel alignment and re pre load the adjuster and tighten lock nut on it. Quite common to find adjusters loose.

After the things that have come loose from mechs on my bikes and cars over the years i cant have faith in the work unless i check it myself-something worked its way loose just the last time i had work done that mech undone and didnt reapply locktight to it like i had-why i have no idea when it could clearly be noticed that it had it on the tread, and i was half way through a ride day when i noticed it. The only thing i didnt check! Serves me right. I recomend to everyone that they check there work done-human error will always be a factor...
